Superior Edge Hockey School (“the school”) shall not be held liable for personal injuries sustained by the student named above. The student assumes full responsibility for any injuries or damages which occur to the student in or about the facilities occupied by the school. The student hereby fully and forever releases and discharges Superior Edge Hockey School, its owners, employees and agents from any and all claims, demands, rights or action or causes of action, present or future, whether the same be known or unknown, anticipated or unanticipated, resulting from or arising out of the student’s use of the facilities occupied by the school or the equipment thereof, including, but not limited to any claims or personal injuries resulting from or arising out of the negligence of the school, the associated hockey rinks, or its owners, employees or agents, or the negligence of any other persons, present on the premises including other persons sincerely using the premises.
I have read this waiver and understand the risks associated with my participation or my daughter’s participation in any activity associated with the school. I further understand that it is required of me to sign this waiver on behalf of ________________________ in order for her to have full participation in the activities of Superior Edge Hockey School.
